How to Replace a Lost or Stolen Canadian Passport
Report the loss to Passport Program authorities, then apply for a replacement with a general application (not simplified renewal) — you need proof of citizenship, a new photo, and a guarantor. Lost or stolen passports are cancelled immediately once reported and can never be reused.

Report the loss or theft
Inside Canada: call 1-800-567-6868 or report at a passport office. Abroad: contact the nearest Canadian embassy or consulate. Reporting cancels the passport immediately in the system, protecting you from identity misuse — do this before applying for the replacement.

Replacements use the full PPTC 153 application (not the simplified renewal): personal history for the past 5 years, references, and the declaration about the lost passport (number, when, where, circumstances).

Arrange your guarantor
Your guarantor must be a Canadian citizen 18+ who has known you personally for 2+ years, currently holds a valid 5- or 10-year Canadian passport, and was 16 when their passport was issued. They sign your photos and the application. No eligible guarantor? A PPTC 132 "declaration in lieu of guarantor" works but adds processing time.

Unlike renewals, replacements require your proof of citizenship (original birth certificate or citizenship certificate — it is returned). Submit at a passport office or by mail. Fees: $120 for 5-year, $160 for 10-year, same as new passports.

I found my passport after reporting it lost. Can I use it again?
No — a reported passport is cancelled permanently and will be flagged at every border. Return it to a passport office or destroy it. Using a cancelled passport is an offence, even innocently.
Does a lost passport affect my replacement application?
One loss is routine. Multiple losses within a short period can trigger additional security review and longer processing. Keep a photo of your passport's data page in encrypted storage so reporting is quick if it happens again.
